At this time, we are seeking a Nurse Practitioner or Physician Assistant to join our Santa Rosa team, helping to deliver comprehensive, patient-centered pain management services in a collaborative and innovative clinical environment.

SUMMARY
The Advanced Practice Provider (APP) provides specialty-level care in the outpatient setting within our pain management practice. Utilizing advanced decision-making skills and clinical expertise, the APP works collaboratively with our physician team to promote optimal patient outcomes.
This position involves comprehensive patient evaluation, diagnosis, treatment, and coordination of care for individuals with acute or chronic pain and musculoskeletal conditions. The APP plays a key role in ensuring Evolve’s high standard of care and commitment to restorative healing at our Santa Rosa location.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Provide direct and indirect patient care in collaboration with supervising physicians
Conduct physical exams, obtain histories, order and interpret diagnostic tests, and prescribe medications as appropriate
Develop, implement, and evaluate individualized treatment plans
Participate in interventional pain procedures as clinically indicated (injections, diagnostic blocks, regenerative therapies)
Demonstrate clinical expertise in pain management, serving as a clinical resource and role model for staff
Coordinate patient care, including communication, triage, lab review, and post-visit follow-up
Document all encounters accurately in the electronic health record
Educate patients regarding diagnosis, treatment options, wellness, and self-care strategies
Maintain compliance with HIPAA, safety, and quality standards
Support clinical workflow to ensure efficient patient throughput and team productivity
Participate in quality improvement and continuing education activities
Requirements
SKILLS, QUALIFICATIONS & EXPERIENCE
Required:
Degree from an accredited institution
Current California Nurse Practitioner license and DEA registration (by first day of work)
Certification by ANCC or ANPP as an Advanced Practice Nurse / Nurse Practitioner or NCCPA as a Physician Assistant
Current BLS and ACLS certification
HIPAA compliance and patient confidentiality training
Proficiency in EMR documentation
Excellent communication, organization, and patient-care skills
Ability to work collaboratively in a multidisciplinary environment
Preferred:
Two (2) years of experience in a clinical setting, but new grads are encouraged to apply
Experience in pain management, neurology, or musculoskeletal medicine
Fluency in both English and Spanish
Strong familiarity with Epic EMR
Prior experience in outpatient procedural settings

WORK SCHEDULE & EXPECTATIONS
On-site in Santa Rosa, CA
Schedule can be Monday through Friday, 20-40 hours each week.
Ability to manage a patient panel averaging 18-24 patients per day (may increase with experience).
Participate in staff meetings, training, and compliance reviews as needed
