Description

Role Overview:

The Pod Provider delivers comprehensive chronic disease management (hypertension, diabetes, chronic kidney disease), acute care, behavioral health condition management (depression, anxiety, substance use disorder), and preventative care. The provider will see all ages. Works collaboratively with an interdisciplinary team to develop member care plans and support their care journey.

Schedule:Monday–Friday: 8am–5pm
No Weekends or Call

Key Responsibilities:

Conduct comprehensive clinical intake visits and provide longitudinal primary or wrap-around care with a focus on chronic condition and preventative care management.
Proactively outreach to members for follow-up and complete comprehensive diagnosis capture (HCC/ICD-10 Coding).
Address quality gaps and ensure proper chart documentation.
Prescribe 1st and 2nd line medications for common behavioral health and substance use disorders.
Remain current on evidence-based care for complex primary care patients.
Participate in case conferences, providing clinical expertise and guidance for care plan development.

Requirements

3+ years of relevant work experience.
Active North Carolina Nurse Practitioner or Physician Assistant License
