We are hiring a Physical Therapist (PT) for a Home Health position providing skilled care to patients in their homes in the Cadillac and surrounding areas and is eligible for $20,000 total bonus potential, including a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year as well as an additional $10,000 sign-on bonus.

What You'll Do:
- Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
- Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence
- Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home
- Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
- Work independently, but never alone
- Travel to patient homes within assigned territory

PT Qualifications
- Active Physical Therapist, PT license in the state you work
- One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care
- Current driver's license and ability to spend ~20% of your day driving to/from patient locations
